This fee is to review costs associated with your requested hospitals. It also consists of a current health insurance review. *Non-Refundable

Once Surrogate Medically
Cleared by IVF Physician

This fee is paid to facilitate the management of your case through the entire pregnancy until three months post-birth.

SURROGATE ELITE Tier Breakdown:

1st 50% due within 5 days of confirmation of pregnancy.

Final 50% due 12 weeks post embryo-transfer.

*All in total includes the application review ($500) and service fee($4,500).

  • $1,500 non-refundable in all cases.
  • $5,000 total becomes non-refundable once confirmation of heartbeat occurs.
  • The Intended Parents must become an association member to access SURROGATE ELITE Surrogacy Care benefits.

Transfer to New Surrogate:

$350 Fee to transfer to a new surrogate. The policy will continue as stated if the transfer is unsuccessful due to miscarriage or loss of pregnancy.

Details:

Maximum: 500,000 (any one pregnancy and in the aggregate)
In-Patient Bed Rest: $1,000 per day up to 30 days (any one pregnancy, pre-authorization required)
Embryo Splits/Twins: $50,000 total.
Fetal Ultrasounds: Limit six per pregnancy.
Fetal NST’s: One per week for singleton, two per week for twins* after 32 weeks gestation. Prior to 32 weeks based upon med. necessity.

Criteria:

  • Surrogates between the ages of 21 and 40; individual applicants greater in age may be eligible at policy terms to be agreed upon.
  • The application form was satisfactorily completed (including Physician Medical Authorization).
  • The gestational surrogate has no adverse medical conditions and/or known or existing and/or prior medical conditions that may be cause for concern of any medical complications related to the maternity and childbirth.
  • Medical services are to be provided in the United States.
  • Pre-certification of surrogate’s maternity, pregnancy, and childbirth-related medical services.
  • Surrogates must have no more than three previous cesarean sections.
  • Surrogates must have no more than four previous pregnancies.
  • No prior history of pre-eclampsia.
  • No prior history of hypertension or pregnancy-induced hypertension requiring hospitalization.
  • No prior history of diabetes or gestational diabetes.
  • No prior deliveries earlier than six (6) months prior to embryo transfer.

Is there any balance billing like other surrogate-specific plans?

No.

What is my deductible for this plan?

Nothing, there is no deductible for this plan.

Is this plan transferrable if I change my surrogate during the process?

Yes, there are some restrictions as to time and if new surrogate qualifies.

What is the Concierge Service?

Plan Concierge Service provides many different benefits including but not limited to finding a physician, scheduling appointments and speaking with our online nursing staff.

What if my surrogate gets put on in-patient physician-ordered bed rest?

The plan covers 30 days of in-patient bed rest.

Will I have co-pays for this Surrogate Maternity Liability Plan?

No.

What if my surrogate loses the pregnancy, will I get a refund?

Yes, depending on where in the pregnancy the surrogate was and according to refund policy.

Do I have to pay extra for the phone application?

No, the app is free to all plan participants.

Will I have to pay for prescription drugs?

No, tier 1-3 drugs are included in your policy.

Will I be able to see the details of my surrogate’s medical appointments?

Yes, as long as your surrogate signs our HIPAA waiver and allows you access to the online portal to see details.
