US based surrogates that have passed the SAGE medical screening, are US citizens and have been working full time (minimum 30 hours per week), for the period of time commencing 180 days prior to the date of application and has not been homebound or hospitalized due to injury or sickness.
TTD: $2,000 per month for 6 Months after 90 days Elimination period and/or;
AD: $500,000 Lump Sum paid in case of the insured Surrogate’s death.
As a result of an accident caused by a complication of pregnancy, the insured suffers a Bodily Injury causing Death. ‘BODILY INJURY’ means identifiable physical injury which is caused by an Accident that is caused by a complication of pregnancy, and solely and independently of any other cause, except medical or surgical treatment rendered necessary by such injury, occasions the death of the Assured within 24 months from the date of the Accident.
Or ‘Accidental’ means a sudden, unexpected, unusual, specific external event, caused by a complication of pregnancy, which occurs at an identifiable time and place during the period of insurance.
Means you are unable to perform the Substantial and Material Duties of your Regular Occupation as a result of an Injury due to complication of pregnancy that first manifests while the coverage is in-force and you are under the care of a Physician appropriate to your condition(s).
The period of time for which the Insured must be continuously disabled before benefits payable. This is found on the Schedule.
The Occupation which you are regularly engaged in. Shown on the Schedule as Surrogate mother.
A condition(s) requiring a hospital stay(s) for an ongoing pregnancy whose diagnoses are distinct from pregnancy but are adversely affected by pregnancy or caused by pregnancy. Non-elective cesarean section, ectopic pregnancy that is terminated, and spontaneous termination of pregnancy occurring during gestation where a viable birth is not possible are also considered complications of Pregnancy. Examples of complications: acute Nephritis, missed abortion and similar surgical conditions. Complications of Pregnancy shall not be defined to include: miscarriage, gestational diabetes, stroke, false labor, spotting, physician prescribed rest, morning sickness, hyperemesis gravidarum, preeclampsia and similar conditions associated with difficult pregnancy not constituting a nosologically distinct complication of pregnancy. The inability to carry another baby, as a surrogate is not a disabling event. This policy does not cover claims in any way caused or contributed to or by sickness or disease or mental infirmity or emotional or psychological trauma.
A miscarriage is not considered a complication of pregnancy, for the purposes of this offer, and is not covered by this policy. The miscarried surrogate will come off risk immediately
